I'm guessing it turns a T-bar into a ratchet. But if that is its purpose, why not just use a ratchet?

Many's the time I've wanted a ratchet head on a 24" Power Bar . . . but I would destroy it .

Ah the pic was just a quick find because i wasn't sure if swivel bar was the correct name , i have a short one in my tool box no longer than a ratchet and its handy when you can't lever at a right angle and the gap is too tight to use a swivel adapter .

Allows a bit of racheting to be done in a confined space when using a long bar or if you don't have a ratchet. Like Shayne says above really.
