Bad case of brake failure by the looks of it… and somebody forgot to tie a knot in the end of the chain!!

Winch man failure is all , its just a big drum brake and if it overheats (which is easily done by stopping and starting as he was) the band (shoe) starts to melt losing all traction .

And you never secure anything that can hold a boat to the bottom even nets worth tens of thousands , to "come fast" as it is known in fishing is an all to common term that has marked the end of days for many a man .
